We are excited to announce an opportunity for a positive and caring professional to join our team as our Clinical Director! *JOB SUMMARY:* In addition to our in-home services, Good Shepherd Homecare & Hospice also owns and operates the Good Shepherd Hospice House, the only inpatient hospice house in the Flint Hills region.
This position is accountable for the coordination of clinical activities for all clients served by Good Shepherd and responsible for assisting the Executive Director in the management of those clinical services.
*ESSENTIAL JOB FUNCTIONS:* 1.
Develops and maintains knowledge of hospice care and compliance with the Hospice Conditions of Participation as a primary responsibility.
Develop and maintain knowledge of home health as secondary.
2.
Provides nursing input towards the initial and updated interdisciplinary plans of care for clients and families.
Part of an interdisciplinary team that completes chart audits to assess the adequacy of the plan of care.
Advocates for necessary positive changes in interventions and the plan of care to meet identified goals.
3.
Supervises the planning, coordination, and delivery of the Interdisciplinary Team interventions.
Participates in weekly team meetings providing leadership and nursing expertise to facilitate problem solving and the delivery of team support in a productive timely manner.
4.
Maintains contact as indicated with Medical Social Services, Chaplain, Physician, Nursing personnel, Aides, Therapies and Volunteers for collaboration to provide holistic care.
Mentors clinical staff to use the full team for optimum patient care and economical use of resources.
5.
Provides nursing input and collaborates to determine the need for special care (respite or general inpatient care, or the need for transfer to facility) and facilitates and assists in a smooth transition from one location or level of care to another.
6.
Is proficient in symptom management and teaches/mentors staff to be responsive to client’s needs, per physician orders and agency policy in a timely, cost effective manner.
Advocates the patient’s right to be as comfortable as possible with consistent management of physical, emotional, and spiritual discomfort by observing and teaching staff and overseeing the patient’s response to the plan of care.

Is an available resource and ensures competency of nursing staff in symptom management, medication administration, and patient care by developing, teaching, and observing competencies, testing, doing supervisory visits, and chart review.

When needed, assists Intake Coordinator in responding to referrals and provides clinical guidance to eligibility and medication questions.
9.
May be required to make nursing visits to prospective clients to evaluate the condition and appropriateness for services.
10.
May provide direct nursing care and support in the field and hospice house, when needed.
11.
Serve as a Liaison between the agency's clinical team and Executive Director as it relates to staffing, policy and procedure and performance improvement.
12.
Develops and maintains relationships with contracted facilities, acting as a liaison between facility and Good Shepherd Homecare & Hospice.
Coordinates the provision of services to patients in these facilities 13.
Interacts effectively with patients and staff through both oral and written communication and supportive presence.
Exhibits effective listening skills 14.
Promotes and contributes to a positive, problem-solving environment.
Maintains a professional attitude and observes proper ethics.
Reviews patient/family/physician/facility satisfaction surveys and uses feedback to make program improvements.
15.
Oversees field on-call to ensure on-call availability 24 hours/ seven days a week.
Works with Scheduling Coordinator to ensure 24 hours staffing coverage at the hospice house.
16.
Provides clinical supervision of nursing personnel and conducts annual performance evaluations.
Acts as a role model, resource and mentor, as appropriate and needed.
Fosters professionalism and staff development and satisfaction.
